Acute Care Hospitals

Hospital records that hold up

Lexcura Summit provides Medical-Legal Analysis for acute care hospitals. See what surveyors, auditors and attorneys would find in your records, and what your clinical, risk and compliance leaders can fix first.

Why it matters

Hospital records span long admissions and multiple disciplines. An ICU stay can run well past 500 pages. Your record must show the risk recognized, the response taken and the follow-through across every handoff. These are the areas we examine:

Falls and pressure injuries

Risk assessments, prevention plans, reassessments and responses that do not align with the recorded injury.

Sepsis and deterioration

Changes in vital signs, labs or condition without a clear record of timely recognition and action.

Rapid response and escalation

Notification, escalation and response timelines that conflict across disciplines or leave unexplained delays.

Medication safety

Orders, administration, reconciliation and monitoring that do not tell a consistent story.

Restraints and informed consent

Orders, monitoring, evaluations and consent documentation that are incomplete or inconsistent.

Handoffs, ED boarding and discharge

Unclear responsibility during transfers, boarding and transitions of care, with gaps in the follow-through.

How we review

Every review follows The Lexcura Clinical Intelligence Model™ through seven pillars, in order, to assess completeness and reliability, establish baseline, measure care against the standard, and map whether a failure connects to injury.

  1. 01

    Record Integrity

Are records complete, reliable, timely and consistent across physicians, nursing, respiratory, pharmacy and therapy?

  2. 02

    Baseline

What was the patient's condition and risk on admission and at each transfer?

  3. 03

    Imaging Analysis

What do recorded imaging and studies show, where they apply?

  4. 04

    Timeline Reconstruction

When did the condition change, and when did recognition, escalation and response occur?

  5. 05

    Standard of Care

How does the documented care compare with what the patient required?

  6. 06

    Regulatory and Compliance Overlay

What do applicable hospital standards, licensure rules and hospital policies require?

  7. 07

    Causation Mapping

Which care or documentation gaps connect to the outcome, and which do not?

Documentation survey readiness

Start with ten charts from one unit or floor you select. We follow each patient’s documented pathway across the hospital, focusing on handoffs and transfers. Findings flag what records show against applicable requirements—not whether your hospital will pass a survey.

Starter

10 charts from one hospital-selected unit or floor. Each patient is followed through their documented hospital pathway, including handoffs and transfers.

$8,950 — one unit or floor

Additional pages: $350 per 100, separately invoiced and paid before review.

Payments cover one hospital. Multi-hospital totals are itemized on a separate invoice and paid before work begins.

Allowances are calculated per paid location and pooled across the operator’s covered locations. Records, event reviews and included claim, notice and audit-letter reviews are quarterly allowances; survey-readiness allowances are annual, not quarterly. Unused allowances do not roll over. Work beyond an allowance, and services not included in the tier, are billed at list price and paid before work begins.

Each hospital documentation survey-readiness review covers 10 charts from one selected unit or floor, following those patients’ documented pathways—not the whole hospital.

Audit-letter reviews look only at what the clinical documentation shows. Lexcura Summit does not provide billing, coding or legal advice.

Hospital standards and review protections

Reviews are measured against applicable CMS Conditions of Participation, Joint Commission standards and state licensure rules, alongside your own policies. Each regulatory overlay is state specific: your hospital is measured against its own state's rules — Florida requirements for Florida hospitals, and each other state's requirements for hospitals in that state.

Reviews can be requested through hospital counsel or your patient safety process. Counsel should determine the appropriate structure and protections before records are shared; requesting a review does not itself establish attorney-client privilege or patient safety work product protection.

How an engagement works

  1. 01

Conflict check and scope: Michelle confirms availability and agrees the records, date range and page allowance with you in writing.

  2. 02

Agreement, BAA and payment: Sign the services agreement and Business Associate Agreement. Pay the full base price upfront; any agreed overages are invoiced separately and paid before work begins.

  3. 03

Secure records transfer: After the agreements are signed, Michelle sends your private portal link. Sign in and upload the agreed complete records and policies there, never by email or through this website.

  4. 04

Review and delivery: Your written report is delivered directly to you, naming strengths, concerns, unfavorable evidence and gaps. Standard delivery is 30 business days after cleared payment and complete records; rush is 20 business days for an additional $1,500, subject to agreed availability.

Who does the work

Clinical findings are made or verified by clinicians holding an active registered nurse license or a higher clinical license.

Trained analysts, including paralegals, organize records, build timelines and check citations under clinical supervision. They may confirm that a date, medication or citation matches the source page; standard-of-care, causation and other clinical judgments go to a licensed clinical reviewer.

Michelle Carroll, Founder and Chief Clinical Strategist, developed the Model and leads each engagement personally.

Confidentiality

We run a conflict check before every engagement and never accept matters adverse to a current client. Records stay in our secure, BAA-covered system, seen only by the team assigned to your work.
