Chart Snapshot: Five Charts, One Clear Picture

You don't need a full audit to learn a lot about how your community documents care. Five charts are usually enough to show the patterns: where notes are strong, where they leave gaps, and which habits would worry an attorney or a surveyor. The snapshot gives you that picture quickly, and if you decide to go further, its fee counts toward the full 30-chart pilot.

What counts as a chart

The most recent 90 days of documentation for one resident: nursing and progress notes, medication and treatment records (MAR and TAR), the care or service plan, physician orders, and incident reports. Therapy notes and outside hospital records aren't included; they can be added at $350 per additional 100 pages.

Every Lexcura review is balanced. Your report names the strengths in your documentation alongside the concerns — including unfavorable evidence and gaps — so you see the record the way an attorney, an auditor or a surveyor would.

Timing and price

Findings delivered within 30 business days after all five charts are received, or 20 business days with rush delivery.

$2,950. Rush delivery in 20 business days is available for an extra $1,500. Paid in full through Stripe before records are sent. Credited in full toward the $15,000 pilot if you sign within 30 days of receiving your findings.

Chart Snapshot: The base price must be paid in full upfront. Any overage is invoiced separately and must also be paid before work begins. Submitting an inquiry does not place a paid order.

After cleared payment and a signed agreement, Michelle sends the next steps. Medical records are shared only through the secure portal after the services agreement and BAA are signed — never by email.
