Single-Event Review: Know Where You Stand After an Incident
A clinical review of one serious event, such as a fall at home or a hospital transfer, with a written risk memo.
$4,500
Pay NowAfter a serious fall at home, a rapid decline, or an unexpected hospital transfer, leadership has two questions: what does our record actually show, and how exposed are we? Lexcura's analysts review the patient's record for that event using The Lexcura Clinical Intelligence Model™ and tell you plainly where the documentation supports your agency, where it leaves you open, and what to fix now.

Scope and record limits
Scope
One patient's record around a single serious event: a timeline built from the record, standard of care and regulatory gaps, and three to five corrective steps.
Record limits
One patient's record, including the first 500 pages. Additional pages are $350 per 100, confirmed with you before the review continues.
Every Lexcura review is balanced. Your report names the strengths in your documentation alongside the concerns — including unfavorable evidence and gaps — so you see the record the way an attorney, an auditor or a surveyor would.
Timing and price
Written risk memo delivered within 30 business days after complete records are received, or 20 business days with rush delivery.
$4,500, including the first 500 pages of records. Pages are counted when the records arrive; anything over 500 pages is invoiced at $350 per additional 100 pages and paid before the review continues. Rush delivery in 20 business days is available for an extra $1,500. Paid in full before records are sent.

After cleared payment and a signed agreement, Michelle sends the next steps. Medical records are shared only through the secure portal after the services agreement and BAA are signed — never by email.
Questions before paying
Please don't include patient names or health information. Records are shared only through our secure portal after your agreement is signed. Lexcura runs a conflict check before every engagement and does not take matters adverse to a current client. This review is a clinical analysis, not legal advice.
