- Reviewed against the standards that apply: the federal Conditions of Participation (42 CFR Part 484 for home health and Part 418 for hospice), your state's requirements and your own policies.
- A findings report: gaps ranked by how likely they are to draw a citation, with a “fix first” list for the next 30 days.
- A one-page staff huddle sheet: the top three charting habits to tighten before surveyors arrive.
- A 45-minute leadership debrief on Zoom to walk through what we found.

10 patient records from one branch. Optional re-check of 5 new records about 30 days later ($1,950). Records over the included pages are $350 per additional 100 pages.

Timing and price
Turnaround is 30 business days from the day we have both payment and complete records.

About 30 days later, we review 5 new records from the same branch to confirm the fixes are holding.
How it works

Submit the request form. We run a conflict check within one business day.

Once cleared, you sign our services agreement and Business Associate Agreement and pay through our secure payment link.

You choose 10 records from one branch and share them through our secure portal, or ask us to choose them the way a surveyor would.

Your findings report and staff huddle sheet arrive within 30 business days, followed by a 45-minute leadership debrief on Zoom.

Be Survey Ready: The base price must be paid in full upfront. Any overage is invoiced separately and must also be paid before work begins. Submitting an inquiry does not place a paid order.
After cleared payment and a signed agreement, Michelle sends the next steps. Medical records are shared only through the secure portal after the services agreement and BAA are signed — never by email.
