Scope and record limits
Scope
One serious event, such as self-harm, a restraint, an assault or an elopement, with a written risk memo. A balanced review through The Lexcura Clinical Intelligence Model™, covering all seven pillars in order: strengths, concerns, unfavorable evidence and gaps, reported plainly.
Record limits
One serious event from one facility. The relevant patient-record set, date range and included page allowance are confirmed in writing before payment and transfer. Additional pages are $350 per 100 pages, invoiced separately and paid before review begins.
Every Lexcura review is balanced. Your report names the strengths in your documentation alongside the concerns — including unfavorable evidence and gaps — so you see the record the way an attorney, an auditor or a surveyor would.
Timing and price
Standard turnaround is 30 business days, or 20 with rush delivery. Reports go directly to you, with an optional 20-minute call to talk them through.
$4,500, paid in full before work begins. Records over the included pages are $350 per additional 100 pages. We count pages before work begins and confirm any extra fee with you in writing.
How an engagement works
- 01
Conflict check: We confirm we're free to work with you, usually within one business day.
- 02
Agreement, BAA and payment: Sign the services agreement, Business Associate Agreement and an additional 42 CFR Part 2 agreement where substance use treatment records are involved. Pay the base price upfront; agreed overages are invoiced separately and paid before work begins.
- 03
Secure records transfer: Michelle sends your private portal upload link after the agreements are signed. Sign in through that link and upload the agreed complete records there, never through this website or by email.
- 04
Review and delivery: Our analysts work through the seven pillars and deliver a written report directly to you.
How to share records
After your conflict check and cleared payment, Michelle sends the services agreement and BAA. Where substance use treatment records are involved, the additional 42 CFR Part 2 agreement is signed before any records are shared.
- Michelle sends you a private secure-portal link after the agreements are signed.
- Use that link to sign in and upload the complete records and agreed policies for your selected service.
- Do not upload records through this website or send patient information by email.
- The record date range, included page allowance and any $350-per-additional-100-page overage are confirmed in writing. Overage invoices must be paid before review starts.

Single-Event Review: The base price must be paid in full upfront. Any overage is invoiced separately and must also be paid before work begins. Submitting an inquiry does not place a paid order.
After cleared payment and a signed agreement, Michelle sends the next steps. Medical records are shared only through the secure portal after the services agreement and BAA are signed — never by email.
